Adjudication form

This page contains links to the form used for sending in adjudication claims. Which one you use depends on how you want to complete and send the form.

1. Click PDF if you want to print off the form, complete it by hand, and send it in by post.

2. Click DOC if you want to open a Word document, complete it on your computer, save it, and send the file by email.

There is no equivalent to the online match result form. This is because the data sent through the form would have to be reconstituted before the form could be given to an adjudicator. This would create more work than it saved.
