Match result form

 

This page contains links to the form used for sending in match results. Which one you use depends on how you want to complete and send the form.

 

1. Click PDF if you want to print off the form, complete it by hand, and send it in by post.

 

2. Click DOC if you want to open a Word document, complete it on your computer, and send the file by email. This document will also open in Pages (for those using a Mac), but it should be saved as DOC or RTF before being sent in.

 

3. Click FORM if you want to complete an online form and send it direct from your computer. Please note: the form contains an anti-spam device. Enter the wrong code and you will get an error message.

The online form is new so some guidance might be helpful. The form is a combination of free-text entry fields and drop-down menus, most of which are self-explanatory. For the Result field:

     •     – – means no decision has been taken about an unfinished game
     •     1–0 is a win for the Home team
     •     0–1 is a win for the Away team
     •     0.5–0.5 is a draw
     •     0–0 is a double default
     •     A–A means the game is going to adjudication.

 

All these Result options are the same as if you are filling out a paper form.

 

If the game is going to adjudication, you need to select a claim from the Adjudication section at the bottom of the form. There are only two options, WIN and DRAW.

Clicking "Submit form" sends the form to both the Senior Recorder and the Web Manager. You will know the form has been sent because you will be redirected to an acknowledgement page.

 

You can print a copy of the form either before or after sending it.
